Cystic lesions of the spleen

Differential diagnosis
The differential for a cystic lesion of the spleen is relatively limited and includes:
- congenital (tend to be unilocular in a majority of cases)
- epidermoid cyst
- cystic lymphangioma
- cystic haemangioma
- post traumatic 'false' cyst
- infection
- hydatid disease
- bacterial abscess
- cystic splenic metastases
- intrasplenic pancreatic pseudocyst
Classification
Splenic cysts can be broadly sub typed into three categories 4
True cysts
- wall calcification uncommon : ≈ 15%
- wall trabeculations / septations common : ≈ 85%
False / pseudo cysts
- wall calcification common : ≈ 50%
- wall trabeculations / septations uncommon : ≈ 15%
Echinococcal cysts
see : hydatid disease
- may show a water lily sign
- may show daughter cysts
