Monteggia fracture-dislocations: Bado classification

The Bado classification is one of the more widely used classifications for Monteggia fracture-dislocations, and mainly focuses on the radial component. Four types are recognised and is generally based on the principle that the direction in which the apex of the ulnar fracture points is the same direction as the radial head dislocation 1-4

  • I: anterior dislocation of radial head
    • classic Monteggia fracture-dislocations
    • this type was originally described by Monteggia in 1814
    • most common type
  • II: posterior dislocation of radial head
  • III: lateral dislocation of radial head
  • IV: anterior radial head dislocation as well as proximal third ulnar and radial shaft fractures
