Unilateral pulmonary edema in severe mitral regurgitation from posterior leaftlet prolapse

Diagnosis almost certain

Presentation

75 year old female. Shortness of breath.

Patient Data

Age: 75
Gender: Female

X-ray at presentation showed right mid zone consolidaiton with thickened minor fissure. Transthoracic echocardiogram performed showed prolapse of posterior leaflet of the mitral valve with severe mitral regurgitation. Patient was commenced on diuretic therapy. 

X-ray the following day showed resolution of unilateral pulmonary edema. 

Case Discussion

Transthoracic echocardiogram showed severely dilated left atrium. Prolapse posterior mitral valve leaflets with anteriorly directed severe regurgitation. Systolic flow reversal observed in the pulmonary veins.  Mildly dilated LV with low normal systolic function, EF of 60%.
