Synovial sarcoma

Presentation

Lump for several months distal femur, just above the left knee.

Patient Data

Age: 7 years
Gender: Male

There is a well-defined hypoechoic lesion with internal vascularity adjacent to the distal femur. Appearances are not of a benign lesion, and further imaging is required for characterization.

Adjacent to the distal femoral shaft is a rounded mass lesion with well-defined borders. It has homogeneous signal characteristics: high-T2 and low-T1.

There is peripheral enhancement and enhancing edema surrounding the lesion. Apperances are concerning for an aggressive lesion, and a biopsy was suggested.

Case Discussion

Imaging findings are of a soft-tissue lesion with non-benign characteristics including internal vascularity. The differential is broad but includes soft-tissue sarcomas like synovial sarcoma or soft-tissue Ewing sarcoma.

A core biopsy confirmed that the mass lesion was a synovial sarcoma.
