Osteoid osteoma - proximal femur

Presentation

The pediatric patient with right hip pain especially in nights, the difficulty of walking due to pain.

Patient Data

Age: 5-10 years
Gender: Male

MR demonstrates focal subcortical nidus isointense on T1WI and mildly hyperintense on T2Fs with surrounding cortical thickening and reactive bone marrow and soft tissues edema in the anterior aspect of the right proximal femoral diaphysis. 

CT shows subcortical nidus with surrounding sclerosis and cortical thickening.

Findings suggestive of osteoid osteoma.

Case Discussion

This is a classic case of an osteoid osteoma. CT was more accurate than MR imaging in the detection of the osteoid osteoma nidus. MR best demonstrates bone marrow and soft tissue changes.
