Presentation

Short history of a distended abdomen and vomiting. Abdominal surgery many years ago.

Patient Data

Age: 65 years
Gender: Female
x-ray

Multiple loops of dilated gas filled small bowel in the central abdomen.

The large bowel is collapsed.   No pneumoperitoneum.

Case Discussion

Multiple features to indicate a high grade small bowel obstruction.

a.  Dilated bowel in the central abdomen

b.  The valvulae connivente are beautifully illustrated indicating it is small bowel.

c.  The large bowel is collapsed.

At laparotomy, this was an adhesional obstruction of the distal small bowel.
