Acute right MCA infarct on CT perfusion

Presentation:

65y old female presented with acute onset left sided weakness and neglect.

Modality: CT

Case Discussion:

Plain CT demonstrates no hemorrhage and hence CT perfusion was done.

CTP shows evolving large R MCA distribution ischemia with almost all core and very minimal penumbra.CBF and CBV of 2.6 and 0.2 respectively to suggest infarct core.

CT Angiography confirms a proximal M1and AI segment of MCA and ACA thrombus in R MCA.There was no dense MCA sign on plain CT.
