Adrenal metastasis from small cell lung cancer - FDG PET/CT

Case contributed by: Dr Chris O'Donnell

Presentation:

Breathlessness in an ex-smoker

Patient Data:

Age: 75
Gender: Male
Race: Caucasian

Typical features of complete left upper lobe collapse

Endobronchial mass producing the lung collapse and a large right adrenal mass.  No other obvious metastatic lesions.

CT PET

Modality: Nuclear medicine

PET/CT for staging of the probable lung malignancy. Marked FDG accumulation associated with the left upper lobe bronchus producing collapse as well as in the large right adrenal metastasis

Ultrasound prior to guided biopsy

Modality: Ultrasound

Solid mass lesion

Case Discussion:

The adrenal is a favoured metastatic site for lung cancer.  Small cell lung cancers typically have large and dispersed metastases with relatively small primaries including lymphadenopathy but also visceral metastases.
