Presentation

Regular attender at the cardiology outpatient department.

Patient Data

Age: 43
Gender: Male
x-ray

Cardiomegaly with a globular outline to the heart.

Upper lobe venous distension, in keeping with pulmonary venous hypertension.

Case Discussion

The heart is enlarged in a globular nature.  The two main differential diagnoses for this appearance to the heart are:

- cardiomyopathy.

- pericardial effusion ( in the acute scenario )
