Choledocholithiasis

Presentation:

Patient with abdominal pain in right hypochondrium. History of surgery to remove the gallbladder.

Patient Data:

Age: 87
Gender: Female
Race: Caucasian
Modality: MRI

Case Discussion:

MRI T2 weighted imaging: filling defects on dilated biliary tree. No gallbladder was seen.   

MRCP (MRI cholangiography) has largely replaced ERCP as the gold standard for diagnosis of choledocholithiasis, able to achieve similar sensitiviy and specificity (approaching 100%) without ionising radiation, intravenous contrast or complication rate inherent in ERCPs.
