Collection post cholecystectomy

Sepsis following cholecystectomy. Any residual collection?

Case Discussion:

Deep to the midline wound but anterior to the anterior abdominal wall musculature is a collection of fluid 12.5cm in diameter, measuring 2.5 cm at its maximal depth. Its wall is thickened and enhances. Associated fat stranding. It contains locules of gas and a further collection of gas lies between it and the wound.

Partial gastrectomy noted. The gallbladder fossa is normal with no mesenteric fat corruption. The remainder of the intraperitoneal space is normal with no free fluid or or active inflammatory process. No collection.

Incidental note of a large lipoma within the left adductor musculature. Moderate right-sided hydrocele.

The collection was subsequently drained under ultrasound guidance.
