The lungs and pleural spaces are clear. Focal eventration anteriorly of the right hemidiaphragm is confirmed on the recent abdominal CT (see below). Heart size is mildly increased. Mediastinal contours are normal. No pneumothorax or free subdiaphragmatic gas. Cholecystectomy clips.

Case Discussion
Eventration of the diaphragm, as in this case, can be large and simulate a mass.
As the diaphragm is intact, a Morgagni hernia is excluded.
