Extensive interstitial opacities bilaterally with cystic ring shadows. Superimposed right perihilar airspace opacification. Possible small effusions.

Extensive cystic changes throughout both lungs. The cysts are predominantly in a subpleural distribution. Background ground glass change.
Case Discussion
As demonstrated on the plain radiograph, this child has extensive lung disease. There are multiple cysts which are predominantly subpleural (peripherally and along the fissures).
Subpleural cysts have been described in the literature in patients with Down syndrome.
