Fatty liver with focal sparing

Case contributed by Dr Franco Ruales

Presentation

Fatty liver disease with focal infiltration

Modality: CT

This case shows a fatty liver with focal sparing.

Case Discussion

Fatty liver can be diagnosed on unenhanced CT if the attenuation of the liver is at least 10 HU less than that of the spleen or if the attenuation of the liver is less than 40 HU. Fatty liver can be diagnosed at contrast-enhanced CT if absolute attenuation is less than 40 HU, but this threshold has limited sensitivity1.
