Presentation
Patient presenting with one day history of left sided headache, had blackout today while driving and hit a small tree, post event amnesia and struggling to find words. PMH: Nil, Nil regular medications.
Patient Data
There is a ring enhancing mass lesion involving left posterior temporal lobe and splenium of corpus callosum, surrounded by moderate vasogenic edema. The lesion shows peripheral margins of restricted diffusion, hemorrhagic products and central necrotic component. The atrium of the left lateral ventricle is compressed, and there is entrapment of the temporal horn. There is subependymal enhancement in the posterior aspect of the body, and temporal horn of the left lateral ventricle. There is mass effect, mild midline shift to the right, no uncal or transtentorial herniation. The lesion is not crossing the midline.
Case Discussion
Imaging appearances are suggestive of a primary high-grade neoplasm, given the patient age and appearance most likely to be a glioblastoma.
