Hilum overlay and loss of more black signs - colorectal carcinoma metastases

Diagnosis probable

Presentation

Cough.

Patient Data

Age: 60 years
Gender: Male

Three pulmonary opacities are visible:

  • left mid zone
  • right lower zone, projecting over the right hemidiaphragm
  • overlying the right hilum - hilar vessels are visible (hilum overlay sign) indicating this is opacity is either in the anterior or posterior mediastinum

Lateral projection confirms:

  • right hilar opacity seen on the PA projection is in the anterior mediastinum
  • right lower zone opacity presents a loss of more black sign indicating its position in the posterior lung base

Case Discussion

CT (not shown) confirmed these opacities as pulmonary masses, presumed to be colorectal metastases.
