The final intercostal catheter (ICC) position is satisfactory but there must have been a low insertion point traversing the liver introducing a small amount of free intra-peritoneal gas and producing a linear low density defect in the liver parenchyma. Fortunately there is no bleeding associated with this iatrogenic hepatic injury.
