Focal parenchymal lesions in the right hemisphere of the cerebellum, left occipital lobe and right frontoparietal area.

The first two lesions present intermediate low signal intensity on T1 and T2 WI. The third lesion has a “cystic” component. Note the peritumoral vasogenic edema on axial T2.

On postcontrast axial T1, nodular compact and clear ring enhancement.

On axial FLAIR, lesions appear moderately hypointense with strikingly hyperintense adjacent edema. No blooming on axial gradient echo.

Case Discussion

Known history of mucinous adenocarcinoma of the colon. The appearance of multiple brain lesions with marginal enhancement consistent with brain metastases.
