Intraspinal synovial cyst

Case contributed by Dr Roberto Schubert

Presentation:

Right low back pain radiating to the L5 dermatoma.

Patient Data:

Age: 47
Gender: Male
Race: Caucasian
Modality: MRI

Osteoarthritis of both facet joints at L4/5 with a large right synovial cyst protruding into the spinal canal and displacing the L5 root.

Case Discussion:

These juxtaarticular cysts do not always possess the signal characteristics of a simple cyst, so IV contrast may be needed in some cases.
