Kyphoplasty – four levels

Presentation

Sudden onset severe back pain with no prior history.

Patient Data

Age: 75 years
Gender: Male

T1 and T2 weighted MRI scan of the thoracic and lumbar spine demonstrates thoracic and lumbar compression deformities at T11, T12, and L2. Marrow edema seen in T12 and L2.

Case Discussion

A 3-level kyphoplasty (T11, T12, L2) was initially planned based on MRI findings. During the planning fluoroscopy scan, it was discovered that L1 had lost height since the MRI. It was then decided to complete kyphoplasty of T11, T12, L1, and L2. T11 and L1 were performed first, then T12 and L2.

A bone marrow biopsy of the right posterior iliac crest was performed after kyphoplasty. Initial results indicate multiple myeloma with hypogammaglobulinaemia.
