Presentation
History of breast cancer post MRM and chemoradiotherpay. Now presenting with delirium and disturbed level of conciousness as well as vomiting and urine incontinence for 2 months.

- diffuse leptomeningeal enhancement with hyperintense FLAIR signal seen smearing the cortical sulci as well as the basal cisterns.
- moderate supratentorial hydrocephalus with transependymal interstitial CSF permeation.
- altered marrow signal of the skull base and calvarial bones with hypointense T1 and T2 signal with heterogeneous post-contrast enhancement.
