Presentation

A 35 year old woman presents to her family physician with a mass lesion at 1 o'clock right breast

Patient Data

Age: 35
Gender: Female

This lesion is described by the radiologist as a complex cyst and called Birads III.

Do you agree ?

No you do not. Not in any shape or form is this a cyst. It even has a blood vessel on Doppler imaging. This is a poorly defined solid lesion with no capsule and its solid, not cystic. This is a Birads IV lesion and biopsy is mandatory after a mammogram is done.

Six weeks later the lesion has doubled in size and biopsy confirms an IDC.

Case Discussion

A cyst should at least look like a cyst.

The intention of Birads III is the lesion is likely being not possibly malignant. There is a difference between the two. Emphasis on benign.
