Case contributed by Amr Farouk
Diagnosis almost certain

Presentation

Accidentally discovered cystic mass on ultrasound

Patient Data

Age: 40 years
Gender: Female

A left adnexal well-encapsulated cystic mass is seen showing peripheral coarse mural calcification, dominant internal fat density (-140 HU) and internal soft tissue mural protuberance. It measures about 6 x 5 cm in its maximal dimensions.

Case Discussion

A cystic mass with fat (areas with very low Hounsfield values), mural calcification (sometimes dentiform), Rokitansky protuberance (solid protuberance projecting from an ovarian cyst) is consistent with a mature ovarian cystic teratoma.
