Extensive, widespread pulmonary airspace opacification, greatest in the lower lobes.

Case Discussion

With a history of prolonged or difficult delivery and these imaging findings, meconium aspiration is really the only differential. The clinical history is usually clear and clinical details will often attest to meconium at delivery.
