Medulloblastoma

Presentation

A young female presenting with progressive headaches and decreased level of consciousness.

Patient Data

Age: 19 years old
Gender: Female

Enhanced brain MRI

The imaging provided shows a large mass obliterating and expanding the fourth ventricle, with upstream changes of acute obstructive hydrocephalus that includes ventriculomegaly and transependymal CSF flow.

The mass is extra-axial, showing a isointense T1 and heterogenous T2/FLAIR signal intensity, with internal foci of acute hemorrhage and cystic changes. GRE images better show the blood products. The diffusion imaging also shows scattered areas of diffusion restriction within the mass.
The post-Gd images show heterogenous enhancement.
 

Case Discussion

Pathology report showed medulloblastoma.

This case shows the typical presentation and MRI imaging findings seen with medulloblastoma.
As the probability of leptomeningeal / CSF seeding is considerable with medulloblastoma, further imaging of the neuraxis was performed and showed no drop metastasis.
