Marked asymmetry of the hippocampal heads with an atrophic right side. Additionally the right hippocampus demonstrates high signal on FLAIR. Features are consistent with mesial temporal sclerosis. 

Case Discussion

The patient went onto have a hippocampectomy. 

Histology

Sections of the mesial structures show extensive mesial temporal sclerosis, represented by neuronal loss in sector CA1, with less mild neuronal loss in sectors CA3 and CA4

Final Diagnosis: right mesial temporal sclerosis.
