Large enhancing mass lesion containing micro-calcification centered on and eroding the inferior right frontal sinus causing it to be obstructed as evidenced by fluid more proximally (arrow).  Note also cerebral parenchymal metastasis (arrow).

Case Discussion

It is very unsual to see metastatic lesions in the paranasal sinuses even when malignant disease is advanced.  The major issue with this disease is the differentiation from a primary adenocarcinoma of the sinus.  In this case the patient has many other metastatic lesions including brain, liver and spinal column.
