Subtle but definite miliary nodules throughout both lungs. These are most noticeable in the right lower zone.

CT chest performed 2 days later confirms the miliary nodules.
Case Discussion
The nodules are visible on the CXR but they can be missed if you are not specifically looking for them. Following the chest x-ray, the patient deteriorated rapidly and they were admitted to ICU. CT MIP imaging reveals the nodules far more clearly.
