Osteosclerosis with facetal arthropathy

Presentation

Past history of operated for spine. complaining of severe backache and foot drop.

Patient Data

Age: 65 years
Gender: Male

Articular processes and posterior arch element shows profound hypointense signals (intensely dark) on T1W and T2W images with facetal arthropathy seen at all lumbar levels s/o osteosclerosis. Sclerosis is also seen at cortical endplates of L3 to L5 vertebral bodies.

Posterior disc herniation and peridiscal osteophyte with facetal arthropathy is seen at L5-S1 level, causing bilateral neural foraminal narrowing, indentation of thecal sac, traversing and exiting nerve roots bilaterally. MR myelography shows block at this level.

Diffuse posterior disc herniations with facetal arthropathy and facetal sclerosis are seen at L2-3 to L4-L5 levels, causing central canal, neural foraminal stenosis, compression over thecal sac, traversing and exiting nerve roots bilaterally. MR myelography shows block at this level. 

Postoperative laminectomy defect seen at L3-4 to L5-S1 levels.  

Case Discussion

Extensive degenerative spine disease with disc desiccation, facetal arthropathy, posterior elements with facetal sclerosis, disc-osteophytes complex with resultant spinal canal, neural foraminal compromise and nerve root compression.
