Anterior knee pain for 3 months, minor trauma?

Age: 25 years

Oblong ossification in region of distal end of patellar tendon.

Ultrasound knee

Intrasubstance fluid cleft in mid line involving proximal - mid third fibers of tendon. Fluid cleft does not involve full thickness of tendon. It does not reach to superficial or deep surface of tendon.

Fibers of medial and lateral aspect of tendon are normal.

Calcification is noted in distal third of patellar tendon fibers.

Deep infrapatellar bursal fluid is noted.

Case Discussion

Findings of intrasubstance fluid cleft with calcification in patellar tendon favor changes of patellar tendinosis without tear.
