Presentation

Painful perineal swelling. Admitted under surgical team.

Patient Data

Age: 50 years
Gender: Female
mri

Thick walled enhancing collection with small internal septations between the anorectum and intriotus.

Strong internal diffusion restriction.

Separate smaller subcutaneous cyst in the right glueteal region.

Case Discussion

An example of a perineal abscess - perhaps overkill to have required an MRI, but nevertheless it demonstrates the pathology nicely.

Likewise the appearances on DWI/ADC in abscess are well shown with avid internal diffusion restriction.

The abscess spontaeneously burst whilst awaiting incision and drainage.
