MR images show increased bulk of the right piriformis muscle compared to the left one.
Case Discussion
Piriformis syndrome is an uncommon cause of sciatica that presents with buttock pain referred to the leg. Diagnosis is usually challenging and is often one of exclusion. In this case, for example, the patient had MRI of the lumbar spine that revealed no significant disc lesions despite his persistent right sciatica and the piriformis muscle belly asymmetry was seen incidentally on an MRI of the sacroiliac joints.
