Known inoperable pancreatic adenocarcinoma. Referred for chest CT due to progressive dyspnea.

Pulmonary lymphangitis carcinomatosa

- Diffuse nodular lesions suggestive of small metastases.
- Marked increase of reticular interstitial pattern, in line with interstitial lung disease.
- A confluent opacity in the right lower lobe.

Disseminated small pulmonary nodules and irregular thickening of interstitial septae, especially in the lower lobes.
Case Discussion
Apart from disseminated small metastases, there is also marked thickening of the interstitial septae due to lymphatic tumor spread.
