Bilateral temporal bone fractures

Presentation

MVA, the patient has hearing problems, the study was done 2 weeks after the trauma.

Patient Data

Age: 50 years
Gender: Male

Right ear shows a longitudinal fracture line crossing the mastoid cavity and bony labyrinth involving the following structures:

  • incudo-malleolar subluxation
  • posterior subluxation  of the stapes relative to the  oval window niche
  • crossing  of the vestibule and the apex of the internal auditory canal
  • dehiscent tympanic segment of the facial nerve canal lying in front of the oval window niche
  • fracture line is also crossing  the round window niche  

Left ear shows a longitudinal fracture line crossing the mastoid  cavity, root of external auditory canal, the  vestibule  and apex of the internal auditory canal:

  • the tympanic segment of the facial canal is dehiscent, lying  in  the oval window niche
  • intra-cochlear ossification  involving the apical cochlear turns (incidental note)
