Testicular torsion

Presentation

Right hemiscrotal non traumatic pain for the last five hours. No fever.

Patient Data

Age: 25 years
Gender: Male
ultrasound

The right testis is enlarged. It shows a normal echo pattern without a mass lesion. Its parenchymal is reduced suggested with increased RI value. There are a twisted spermatic cord showing concentric pattern on the grayscale as well as with color doppler scan. The right epididymis showed mild edema. There is a mild reactive hydrocele. Left testis and epididymis show a normal echopattern and vascularity. Scrotal wall thickness is normal.

Case Discussion

A young male presented with acute onset of right hemiscrotal pain without fever/ trauma. Ultrasound shows a whirlpool sign which is a primary sign of testicular torsion. There are right testicular edema and reduced parenchymal vascularity which are the secondary signs of the testicular torsion.

Surgery was done within a few hours. The right testis was viable. It was repositioned. Bilateral orchidopexy was done.
