Transient bone marrow oedema - hip

Case contributed by: Dr Chris O'Donnell

Presentation:

Trivial "injury" at work, now severe right hip pain for months (continued working)

Patient Data:

Age: 47
Gender: Male
Modality: MRI
Modality: Nuclear medicine

Case Discussion:

Diffuse bone marrow oedema throughout femoral head and neck with features of synovitis.  Bone scan shows focal hot spot in the femoral head with subtle linear defect on T1 coronal MRI = subtle "stress" fracture with marked surrounding reactive oedema.   This is thought to be the origin of so-called "transient bone marrow oedema".
