Multifocal collapse secondary to underventilation when intubated

Diagnosis almost certain

Presentation

Bronchiolitis. Intubated.

Patient Data

Age: 3 years
Gender: Male
Race: Caucasian

Multifocal collapse in the upper lobes bilaterally (greater on the right) with subsegmental collapse at the right base and extensive peribronchial thickening centrally.

Increase in ventilator settings

ETT and NGT in appropriate positions.

The lungs are well expanded and there is no evidence of consolidation, collapse or pneumothorax. There is mild peribronchial thickening centrally.

Case Discussion

The rapid change following an increase in ventilator pressure settings proves that the opacification demonstrated on the initial radiograph was secondary to collapse, not consolidation. It is interesting how the prominence of peribronchial thickening also reduces.
