Spiral fracture of the distal fibula at the upper level of the syndesmosis with widening of the syndesmosis. Displaces fracture of the medial malleolus below the level of the tibial plafond. Fracture of the posterior malleolus. There is slight lateral subluxation of the talus relative to the tibia.
