The piglet sign is seen in central pontine myelinosismyelinolysis. It refers to appearance of upper pons in axial T2 and FLAIR images. If the areas of demyelination coalesce, axial MR images resemble the face of a piglet with the temporal lobes representing ears, the carotid arteries representing  the eyes, the area of central pontine myelinosis representing snout and the fourth ventricle as the mouth 1.
