A canal-wall-down mastoidectomy comprises of a group of mastoidectomies which is more open and extensive than a canal-wall-up mastoidectomy. In addition to traditional forms, various modified formforms are now performed (see modified canal wall down mastoidectomy).
They initially comprise the similar steps as in canal-wall-up mastoidectomy, but in addition the posterior wall of the external auditory canal is resected to increase exposure to middle ear contents and the Koerner's septum is removed. The eustachian tube is obliterated to completely isolate the middle ear 3.
