Cystadenocarcinoma of the rete testis is the malignant version of cystadenoma of the rete testis.

Epidemiology
Primarily a disease of older men (>60 years old), but has been noted in a wide age range (8-91 years).
Pathology
Histologic evaluation requires exclusion of malignant mesothelioma or papillary serous carcinoma, which can be mimics.
Radiographic features
Cystadenoma and cystadenocarcinoma of the rete testis cannot be radiologically distinguished, and prospective diagnosis of cystadenocarcinoma does not currently seem possible.

~25% of tumors are associated with hydrocele.
Treatment and prognosis
Treatment and survival data are not robust due to the rarity of the tumor. Post orchiectomy patients still carry a guarded prognosis, unlike cystadenoma.
Differential diagnosis
Radiologic differential diagnosis includes
- dilated rete testis
- metastasis to the testis: rare, but more common than cystadenocarcinoma
