Fleck sign (ankle)

The fleck sign in the ankle refers to a cortical avulsion fracture of the distal fibula (lateral malleolus), indicating superior peroneal retinaculum injury. This fleck sign should raise the possibility of peroneal tendon subluxation or dislocation 1,2, which commonly accompanies intra-articular calcaneal fractures 3.
