An incompetent ileocecal valve is a situation where there can be reflux of backward flow of food content from the large bowel (cecum) through to the small bowel (terminal ileum) and through the ileocecal valve. A low degree of incompetence is not an uncommon finding 3. In some states, patients may decompress their large bowel into the small bowel and present with vomiting although its presence may reduce the rate of ongoing distension secondary to an obstructed colon.
