When a pleural effusion is large and unilateral, concern for an underlying abnormality should be raised. Causes include:

extension from subdiaphragmatic primary infection

ruptured / injured thoracic duct
tumor infiltration e.g. lymphoma

hemorrhage
trauma
iatrogenic trauma

cardiovascular / fluid related causes
left ventricular failure (unilateral uncommon)

liver cirrhosis - hepatic hydrothorax
