Lichtman classification of Kienböck disease

The Lichtman classification is the most used classification system for assessing Kienböck disease and is used to guide management and assess prognosis. It is based on the radiographic features of the lunate and adjacent carpals and radius.

Classification

  • stage 0: prenecrotic

  • stage I: normal radiograph, osteonecrosis noticeable on MRI or bone scan

  • stage II: lunate sclerosis

  • stage III

    • IIIA: lunate collapse, radioscaphoid angle <60º

    • IIIB: lunate collapse and carpal instability, radioscaphoid angle >60º

    • IIIC: chronic coronal lunate fracture

  • stage IV: carpal collapse and perilunate arthrosis (Kienböck disease advanced collapse)
