Metastases usually spread from contiguous sites most commonly, with lymphatic and hematogenous metastases also recognized.

The location of the metastases that are adenocarcinomas can help determine the origin and are predominantly from the:
- upper genital tract if located within the anterior wall of the upper-third of the vagina
- gastrointestinal tract if located within the posterior wall of the lower third of the vagina
