The modified Dallas classification describes the discography appearance of radial annular tears of the intervertebral disc.
Classification
grade 0: contrast remains in the nucleus pulposus
grade 1: contrast leaks into the inner 1/3 of the annulus fibrosus
grade 2: contrast leaks into the middle 1/3 of the annulus fibrosus
grade 3: contrast leaks into the outer 1/3 of the annulus fibrosus
grade 4: grade 3 tear with contrast spreading circumferentially (at least 30o) within the annulus fibrosus
grade 5: grade 3 or 4 tear with contrast spreading outside the annulus fibrosus (extradiscal rupture), a.k.a. evil tear
