Paediatric appendicitis score

The Paediatric Appendicitis Score (PAS) is a clinical decision rule and predictor of the likelihood of acute appendicitis in the paediatric population 1.

Criteria

  • cough/percussion/hopping tenderness in right lower quadrant (+2)
  • anorexia (+1)
  • fever (+1)
  • nausea or emesis (+1)
  • tenderness in right iliac fossa (+2)
  • leucocytosis (+1)
  • polymorphonuclear neutrophilia (+1)
  • migratory abdominal pain (+1)
Interpretation
  • 0-5 incompatible with acute appendicitis
  • 6-10 compatible with acute appendicitis

Scores greater than 7 indicate high probability of appendicitis 1. PAS system has reported sensitivity of 0.87-1 and specificity of 0.59-0.92 1,2.
