Pool sign (intracranial; metastatic adenocarcinoma)

The pool sign describes the brain MRI appearance whereby an intracranial mass exhibits a T2 hyperintense rim adjacent to a solid mass surrounded by peritumoral edema. This appearance has suggested to be a feature characteristic of metastatic adenocarcinoma (with various primaries).

The "pool" part of the lesion was found to be iso/hypointense on T1 weighted imaging in comparison with the gray matter, while isointense with vasogenic edema on T2 FLAIR sequences 1,2. The theoretically suggested causative factor of this phenomenon is the leakage of secretions from the solid component of the lesion 1.
