The Pubofemoral distance (PFD) is a measurement used to detect hip instability and avoid the late diagnosis of developmental dysplasia of the hip. It is a reproducible and straightforward sonographic screening test that measures the distance between the medial margin of the epiphysis and the pubic bone 4.

A pubofemoral distance greater than 6 mm (at the age of 1 month) or a difference greater than 1.5 mm should trigger referral 1.

While PFD measurement is accurate for diagnosing DDH, it should not be considered a replacement for the Graf method due to its low-positive predictive value 2.

The pubofemoral distance increases with age but is not significantly influenced by laterality or gender. This means that it can be used as the follow-up of infants treated for DDH 3.

The pubofemoral distance is a valuable tool for detecting hip instability and preventing potential complications, allowing for early intervention and treatment.
